What information does the form ask for?<\/h2>\n
You wil be asked for your PPS number, Signature and Date.<\/p>\n<\/div>\n

Do I have to divulge my earnings?<\/h2>\n
You do not have to tell us how much you earn.<\/p>\n<\/div>\n

Do I have to fill out this form every year?<\/h2>\n
The CHY3 form is valid for five years, cutting down on administration costs and making your donation more effective. This does not mean that you have to make a donation every year, but if you decide to do so, we can claim the tax on all donations over \u20ac250 in any one tax year.<\/p>\n<\/div>\n
