What we do?
- Support service users to attend, advocate and understand health appointments.
- Support service users to find out about their health and health services.
- Promote the rights of the service user.
- Give support in attending follow up health appointments.
- Complete a handover with the service user’s key workers before/ after appointment to relay any important information.
Who is the service for?
The service is designed for people who are currently experiencing homelessness and in need of advocacy/accompaniment support to their health care appointments.
Cork HHPA Case Management Support Service
Criteria: The person is experiencing homelessness; health concerns and complex support needs.
Support Service: Conduct assessments, create support plans, referrals to services and Case Management support. Work directly with service users using a case management approach to access necessary services; advocating for service users. Engage and coordinate support services across community, hospital, and accommodation sectors for Inclusion Health clients experiencing homelessness with complex health, addiction, and/or mental health challenges.
